Healthcare CEMP

Patients and residents rely on their health facility to provide a high level of care, even during an emergency. A Comprehensive Emergency Management Plan (CEMP) helps health care facilities face all kinds of threats and keep their clients safe.

Santa Rosa County Emergency Management has the statutory responsibility of reviewing and approving the CEMPs based on the criteria established by the State of Florida Agency for Healthcare Administration (AHCA). Here is a list of Emergency Plan Regulations and Review requirements.

Facilities Review and Submission Requirements

Santa Rosa County has different criteria for CEMPs depending on the type of facility. The required documents must accompany the submission for the CEMP to be accepted for review. The state’s CEMP forms can be found on the AHCA website.

Assisted Living Facilities

• Full CEMP
  • Completed CEMP Template (must be updated annually)

    Nursing Homes

    • Full CEMP
      • Completed CEMP Template (must be updated annually)
        • Our office will not accept partial submittals or submittals of page updates. If you use this template, the above crosswalk is not needed.

        Hospitals

        • Full CEMP
          • Completed CEMP Template (must be updated annually)
            • Our office will not accept partial submittals or submittals of page updates. If you use this template, the above crosswalk is not needed.

            Ambulatory Surgical Care Centers

            • Full CEMP
              • Completed CEMP Template (must be updated annually)

                *AHCA Criteria Cross-Walk
                The crosswalk documents identify the minimum criteria to be used when developing a CEMP for residential health care facilities. The crosswalk serves as the required plan format for the CEMP, and will also serve as the compliance review document for Santa Rosa County Division of Emergency Management upon submission for review and approval pursuant to Chapter 252, Florida Statutes (F.S.). These criteria satisfy the basic emergency management requirements of 400, Part II, Florida Statutes, but are not designed to provide specific emergency medical planning guidance.
